Are you a passionate and experienced Practice Nurse with a special interest or qualification in diabetes care? We're looking for a dedicated Diabetes Specialist Nurse to join our growing team and make a real difference in the lives of our patients.

About the Role:

You'll lead on diabetes management and patient education, working closely with our multidisciplinary team to deliver high-quality, patient-centred care. This is a fantastic opportunity to play a pivotal role in developing and improving chronic disease management pathways in a supportive and innovative environment.

Key Responsibilities:

  • Provide expert diabetes care and support for patients
  • Run nurse-led diabetes clinics and support lifestyle interventions
  • Collaborate with GPs, pharmacists, and healthcare assistants
  • Contribute to service improvement and QOF outcomes

What We’re Looking For:

  • NMC-registered Nurse with post-registration experience
