Diabetes Specialist Nurse- Prescriber Desirable

An opportunityhas arisen for an enthusiastic, professional and experienced Diabetes Nurse ideally with advanced certification in delivering the management ofdiabetes.

StoursideMedical Practice is a forward thinking 6000 patient GP practice in the centreof Halesowen. We are close to the West Midlands motorway network and on busroutes from Birmingham and Wolverhampton. We are based in a modern healthcentre with associated community services under one roof. We are a TrainingPractice and have an experienced team of clinical and non-clinical staff whichinclude a paramedic, a First Contact Physio, First Contact Mental Healthworker.

We are lookingfor someone to work 1 day a week to support with the management of our diabetes patients. This post would suit a nurse who is currently working in a hospital or in community and looking for variety one day a week. The candidate would have experience of insulin initiation and GLP1s. We also pay an enhanced rate for extended access shiftswhich are currently available to work if desired by the successful candidate.

Main duties of the job

MainDuties of the Job

Thesuccessful candidate will need experience of working with diabetic patientswhether that it is hospital/community setting or working within generalpractice. They would help manage more complex diabetes patients who may need insulin initiation or titration as well as conducting the routine annual reviews with all diabetic patients.

Job responsibilities

Managing and treating our 400 diabetes patients.

Initiating insulin and GLP1's where indicated

Annual and bi-annual diabetes reviews

Raising safeguarding concerns whenneeded.

To maintain a clean, safe environment,adhering to hygiene and infection control.

Provide home visits for the houseboundpatients needing support for their long term conditions

To select patients for, and attend aquarterly diabetic MDT with a hospital consultant and the GP diabetic lead
