Overview
We are recruiting a Band 7 Diabetes Clinical Specialist Nurse to work within Sefton Place Community Diabetes Team. The post holder will demonstrate a comprehensive knowledge base in relation to Diabetes and promote excellence in nursing practice in a dynamic healthcare environment. The post holder will demonstrate leadership values within the team and contribute to delivery of specialist nursing care, case management and direct patient care, acting as an expert practitioner to optimise patient health, provide education for self-care and complex management of patient pathways. The post is based in Sefton Place and provides a service to patients in Southport, Formby and the South Sefton area.
The Specialist Diabetes Nurse will be part of a multi-disciplinary team and adapt an integrated approach to care delivery and service transformation.
Responsibilities
- Deliver specialist nursing care, case management and direct patient care as an expert practitioner to optimise patient health.
- Provide education for self-care and complex management of patient pathways.
- Lead and contribute to the delivery of the service within the area, including leadership duties in the absence of the Team Leader.
- Work autonomously, demonstrate evidence-based clinical decision making and maintain clinical credibility by supporting team members in the clinical environment.
- Provide clinical advice and support to service users and the wider healthcare team, including clinical assessment, diagnostic skills, non-medical prescribing and implementation of management and treatment plans.
- Collaborate with other services/stakeholders to ensure service users are treated in the most appropriate setting and maintain a high-quality service through caseload and clinical supervision.
- Develop and manage caseload across the local health economy; provide leadership and mentoring to staff developing into caseload management roles.
- Act as an advocate for patients/clients in forums and professional groups, challenging attitudes and behaviour where needed.
- Plan rotas and working patterns to ensure business continuity; practice autonomously and support practice development and service modernisation.
- Assess patient conditions, develop specialist individualised care plans with patient/carer involvement, and implement and evaluate care delivery for identified needs.
- Ensure clinical activity reflects health promotion, supported self-care, disease-specific management and long-term condition care, including end of life/palliative care where applicable.
- Set objectives and participate in local service delivery meetings; provide reports on staff and patient activity as requested.
- Collaborate with others to support policy development, service redesign and mentorship/clinical supervision within the team.
- Ensure induction for new staff and students; oversee timecard and mileage authorisation as an authorised signatory.
- Monitor budgets, participate in PACE reviews, and report on service performance and contract objectives.
- Ensure delegation of administration and clerical duties to support staff; contribute to audits, research and supervision; maintain clinical competencies and records in line with professional standards.
- Support the identification of skill deficits within the team and address them to support service delivery and development.
- Contribute to the development of local policies and services to reflect local needs and participate in policy/service development.
