Bradford is the 5th largest metropolitan borough in England, with a vibrant and highly diverse population. With probably the highest rates of diabetes in the UK, you will never be bored working in diabetes in Bradford.
We are looking for people who wish to develop into senior specialist diabetes nurses, working across primary, community and specialist secondary care services, including a large technology service, who have experience working in diabetes and which to progress their career further.
The post holder will work as part of the diabetes integrated multi-disciplinary team providing clinical support to community based intermediate diabetes care clinics, specialist diabetes clinics, and the in-patient population with diabetes. The post-holder will be responsible for the management of a defined caseload of patients with Diabetes, providing specialist nursing support, advice, education and/or care to that specific group. They will also participate in the delivery of structured patient diabetes education sessions and healthcare professional education both for community and hospital-based staff.
The post holder will work as part of innovative diabetes nursing team, supporting the implementation of Think Glucose, a national initiative designed to improve the care of in-patients with diabetes, across Bradford Teaching Hospitals Foundation Trust (BTHFT)
