Qualifications

  • Level 1 NMC Registration is required.
  • Minimum 3 years post registration experience in clinical diabetes.
  • Competent in both inpatient and outpatient settings.

Job description

We are looking for one Band 7 Diabetes Specialist Nurse to join our team at Imperial College Healthcare NHS Trust (ICHT), which is comprised of five acute hospitals. Working as part of a multidisciplinary team with consultants, podiatrists, dieticians and other professions, the diabetes nursing team perform a crucial role in making a difference to patients with diabetes, frontline staff and a service provider in Northwest London.

The post-holder will be required to cover any of the other sites in the event of an emergency though will be based at Hammersmith Hospital site.

The post is required to provide a 5-day service from Monday to Friday on site.

Main duties of the job

o To provide sound clinical leadership and develop support mechanisms for sharing good practice within the clinical setting and Trust wide.

o Maintain and promote effective communication both verbally and in writing with all members of the multi-disciplinary team and other Wards or Departments, including formal and informal presentation of information relating to your service.

o To work with multidisciplinary team to implement National Service Framework for Diabetes, NICE guidance, local guidelines and GIRFT recommendations.

o To support Senior Diabetes Specialist Nurses and ward staff to develop diabetes care for inpatients with diabetes.

o Ensure any information/data required for clinical audit or statistical analysis or for research, projects or registers are recorded and collected correctly.

o Working cross -site within the Trust, if required, to provide cover for colleagues and maintain service provision that is safe and of a high quality.

Person Specification
  • Level 1 NMC Registration
  • Teaching Certificate or experience as a preceptor/facilitator/mentor.

Experience
  • Minimum 3 years or more post registration experience in clinical diabetes
  • Previous experience as a Band 6 or 7 DSN
  • Competent and able to work with patients in both inpatient and outpatient environments
  • Ability to run own outpatient clinic and work in clinic as part of a MDT
  • Understands and holds relevant experience of delivering inpatient diabetes support and education.
  • Self-directed able to generate new idea is to develop and improve services.
