Qualifications

  • Minimum 3 years' experience managing diabetes within primary care.
  • Knowledgeable in diabetes patient education and care.
  • Skills in oral therapy optimisation and IT literacy in primary care systems.

Job description

Are you an experienced Practice Nurse with Diabetes qualifications, seeking your next challenge?

Would you like to make a real impact in Diabetes care, alongside a dynamic team in Buckinghamshire, Milton Keynes or Bedfordshire?


We are working alongside an independent health provider who are growing their nationwide team of nurses working in primary care, delivering a Diabetes service to patients.

You will be committed to delivering the highest quality care to improve patient outcomes, in line with local and national Diabetes guidelines (NG28).

Working in a pro-active manner, you will be identifying sub-optimally controlled patients with Type 2 diabetes, optimising their oral therapy, conducting point of care testing and referring for specialist insulin treatment where necessary, whilst exercising corporate nursing responsibilities.

Essential

  • Diabetes diploma or equivalent ENB qualification
  • A minimum of 3 years' experience (post diabetes qualification) managing diabetes within primary care
  • Knowledgeable and confident in the provision of education, support and advice to people with Type 2 Diabetes
  • Skills and knowledge in the optimisation of oral hypoglycaemics
  • IT literacy, to include primary care systems

Schedule: Monday to Friday, no Bank Holidays
