To participate in the coordination of patient care to ensure continuity throughout the patient’s care pathway. To be pivotal in review and development of the T1DE service, and to reflect the needs of the patient group.
  • To liaise closely with the health care teams of inpatient wards, outpatient department and primary care, including Consultant Physicians and GP’s. To demonstrate wide and in-depth experience of all aspects of the speciality, exercising clinical leadership and effective education within their sphere of practice. To monitor and improve standards of care through clinical audit, leading the development of practice through research.
  • To lead on the delivery of Diabetes Specialist Nursing care within the hospital, predominantly, as part of the T1DE project MDT, providing clinical leadership and support, development of nurse-led protocols/guidelines and nurse-led initiatives.
  • To develop and lead in the collection of relevant audit data to facilitate quality improvement, service development and national requirements.
