Main responsibilities
- The post holder will work as part of the diabetes integrated multi-disciplinary team providing clinical support to community based intermediate diabetes care clinics, specialist diabetes clinics, and the in-patient population with diabetes. They will manage a defined caseload of patients with Diabetes, providing specialist nursing support, advice, education and/or care, and will participate in the delivery of structured patient diabetes education sessions and healthcare professional education for both community and hospital-based staff. Clinical care will be delivered at numerous primary and secondary care locations across Bradford.
- They will work as part of an innovative diabetes nursing team, supporting the implementation of Think Glucose across Bradford Teaching Hospitals Foundation Trust (BTHFT). They will carry continuing responsibility for the assessment of needs, development, implementation and evaluation of programmes of care and the setting of standards within BTHFT.
The diabetes specialist team delivers a range of patient orientated structured education sessions, many accredited by a national scheme (Qismet). The post holder will take an active part in this service and will take daily responsibility for managing a defined caseload, assessing patient needs and delivering individualised care and treatment for people with diabetes.
The post holder will participate in providing clinical care and education across both primary and secondary care settings and will support the implementation of national initiatives to improve inpatient diabetes care.

Experience
- Essential: Significant post-registration experience, including experience of working in a relevant clinical specialty
- Essential: Experience of assessment, development, implementation and evaluation of programmes of care for patients/service-users/staff
