Job Summary

We are currently looking for a diabetes practice nurse to help manage our complex diabetes patients.

The role is for 10 15 hours per week, with flexibility on days and times, and would be suitable for remote working. The post is for an initial trial period of 6 months fixed term, after which the role will be reviewed with the possibility of it becoming a permanent position. The ability to offer occasional home visits would be desirable, but not essential.

The post holder will work autonomously and as part of the multidisciplinary team to deliver high-quality, evidence-based care, ensuring patients receive personalised treatment and education to support safe, independent diabetes management.

Salary: £22.00 per hour.

Main duties of the job

The post holder will be responsible for their own caseload of diabetic patients, developing, implementing and reviewing personalised care plans, including initiating and adjusting insulin doses.

They will monitor medication effectiveness, ensuring treatments are safe, evidence-based, and meet individual needs.

The role involves providing practical and emotional support to patients, particularly those newly diagnosed or experiencing complications.

They Will Complete Referrals To Other Services As Required.

Accurate record-keeping is vital, alongside involving patients and carers in planning care. The post holder will work closely with GPs, nurses, HCAs, pharmacists and external partners to ensure continuity. They will also act as a clinical resource, support staff development, provide patient education, and promote self-management, prevention and early intervention.

The post holder must become familiar with enhanced service specifications regarding diabetes and ensure they work within those specifications and assist the practice in providing any monitoring requirements.

St Georges Medical Practice is a friendly, 10 doctor, practice providing care for over 15,000 patients based in the centre of West Bridgford. We are an established teaching and training practice, accommodating medical and nursing students and GP Registrars. We have a multi-disciplinary practice team with care also being provided by First Contact Physios, Pharmacists and a Social Prescriber. We aim to provide high quality, safe and compassionate care in partnership with our patients and seek to continuously improve the service we offer.

Education And Experience

Job Summary

To work in conjunction with or independently of the other members of the practice team, according to practice policy and protocols. To deliver high-quality, evidence-based care, ensuring patients receive personalised treatment and education to support safe, independent diabetes management, whilst maintaining professional conduct in accordance with NMC standards.

Principle Duties

Professional

  • Develop, implement and review personalised care plans for diabetic patients.
  • Perform insulin initiation and adjustments to doses as required.
  • Monitor diabetes medication effectiveness, ensuring treatments are safe, evidence-based, and meet individual needs.
  • Provide practical and emotional support to diabetic patients, particularly those newly diagnosed or experiencing complications.
  • Complete referrals to other services as required.
  • Be familiar with monitoring and reporting requirements for diabetes, including enhanced service specifications.
  • Liaise with other members of the primary health care team and other agencies accordingly, to maintain provision of appropriate care.
  • Ensure all nursing care, interaction and procedures are recorded accurately and within 24hours, into patients medical records according to practice protocol, including opportunistic QOF requirements.
  • Provide audit information in accordance with practice policy and the Care Quality commission (CQC).
  • Work according to Health and Safety regulations as specified in the Health and Safety policies.
  • Maintain confidentiality at all times with regard to both patients and staff.
  • To provide support to colleagues with diabetic follow up.
