A local health initiative is seeking a Practice Nurse to support diabetic patients. This role involves delivering safe nursing care, managing treatment plans, and providing education to patients. The ideal candidate must be NMC registered with a passion for holistic care. Operating in a supportive environment, you will collaborate with a dedicated team to enhance patient wellbeing in Wareham, England.
Weare looking for a Practice Nurse who has a special interest in Diabetes to joinour friendly and proactive dispensing practice for 10 hours a week.
The successful candidate will deliver safe and effective nursingcare to our patients and working in accordance with the NMC. The post holderwill provide a holistic approach to patient centred care and with a particularinterest and / or experience in managing patients with Diabetes.
You will be responsible for reviewing their treatment,blood test results and providing education and health management to optimisetheir health and wellbeing.
We have an excellent, dedicated and loyal team working from purpose-built premises with a list size of approximately 6000 patients. We use TPP SystmOne. Working with 6 GPs, 3 PN's and 3 HCA's.

Job Summary:
The post-holder will work with reference to Practice agreed protocols and guidelines to provide diabetic nursing support to patients both independently and in participation with nurse colleagues and general practitioners.
The post-holder will maintain the required level of training and competence and will work within patient group directions where these are available.
The post holder will liaise where necessary with the Department of Diabetes at Poole General Hospital OR Dorset County Hospital.
