Job description

Diabetes Nurse Specialist Leading Transition & Education

Works autonomously to support the management of the transition caseload of patients while working as part of a multidisciplinary team. Leads the preparation for transition nursing clinic. In the absence of the team leader, manages the diabetes nursing team. Provides a non-medical prescribing role or works towards it. Facilitates good communication links between children or young people with diabetes and their carers or others involved in management. Handles calls and queries, communicating plans to the child/young person, their carer, or other professionals as appropriate.

Provides education and information to children, young people, and their families or carers on diabetes-related issues, such as management, lifestyle, safety, and school considerations to prevent hospital admissions. Offers clinical advice, support, and training to community healthcare professionals, education, and social care teams. Provides training for schools and supports ongoing education and healthcare plans for individual children. Advises and assists families, initiating early help referrals to social services as needed.

Enables children, young people, and their carers to participate in decisions about transition care. Coordinates patient care and transition pathways, liaising across organizational boundaries. Plans specialized transition programs and offers age-appropriate advice to children, young people, relatives, and carers. Provides clinical advice based on current evidence-based practice.

Participates in nurse/consultant-led clinics, multidisciplinary team meetings, case conferences, and MDTs related to future management or transition. Ensures families receive verbal and written information about the transition process. Maintains accurate records and documents all relevant information in medical and nursing records.

Managerial/Leadership: Manages own caseload, coordinates the transition journey across divisions and boundaries, supports the diabetes lead nurse, and manages the team in their absence. Collaborates with other healthcare professionals to meet transition needs. Acts as a mentor to pre- and post-registration nurses, assisting with their learning. Develops policies, protocols, and procedures aligned with standards, especially regarding transition.

Education and Development: Participates in training and education for children, young people, carers, and healthcare professionals involved in management. Supports education and social care development, provides clinical supervision, and maintains educational packages. Keeps skills and knowledge current through ongoing professional development, training, and conferences. Ensures mandatory training is completed and promotes health within the specialist area.

Clinical Governance: Ensures compliance with policies, procedures, and guidelines. Monitors care standards, participates in audits and research, and promotes service improvement. Advocates for patient and public involvement and upholds principles of equality, diversity, and rights.
