Job summary
The post holder will be part of our team of Diabetes Nurse Specialists delivering a comprehensive, high-quality evidenced-based Diabetes Specialist Nursing service to a designated population within Powys. They will collaborate with Multi-disciplinary Teams across Primary, Community, and Secondary Care along integrated pathways of care. The role involves contributing to the planning and development of Diabetes services across Powys within the team, providing training and development to PTHB staff and partners within the Community Resource Team on diabetes management, treatment, and care.

Our Diabetes Team is small, friendly, and passionate about care. We have 22.5 hours available to cover across the county, primarily in South Powys, including in-patient, out-patient, and community services, with virtual and face-to-face consultations. This role is ideal for a practitioner wishing to progress in their diabetes nursing career.
We value team players with experience and knowledge in diabetes who are motivated to enhance the service. The role requires advanced practice skills, public health promotion, and an understanding of the social, economic, and political healthcare context.

Qualifications include NMC registration, nursing degree, postgraduate specialist training in Diabetes, and teaching qualifications. Desirable qualifications include Independent Prescribing, MSc in Advanced Clinical Practice, and management/leadership qualifications.
Experience required includes management-level service improvement and clinical assessment of respiratory patients. Essential skills include computer literacy, patient assessment, communication, teamwork, professionalism, motivation, and change management. Welsh language skills are desirable.
