We are looking for a Practice Nurse with a special interestin Diabetes

An exciting opportunity has arisen to join our friendlyteam serving 21,300 patients from our GP practice in Stratford upon Avon. We are an excellent forward thinking practicethat have a large and well supportedclinical team

You will be supporting the GP's and Nurse Partner in thedelivery of nursing care, including diabetes, to our patient population.

Ideally, you will have previous experience of diabetes careas part of chronic disease management within Primary Care. Alternatively, youmay have a special interest in diabetes and be looking to quickly develop yourskills in this area. We will consider applicants with special interest in other areas of nursing

Experience in managing the type of minor illnesses thatpresent in Primary Care would also be advantageous in your application.

To work within our current nursing team to deliver a highstandard of clinical care to patients. To assist with our Chronic DiseaseClinics. The ideal candidate will have a special interest in Diabetes

Job responsibilities

PrimaryDuties

Your main role will be to implement andevaluate individual treatment plans for chronic disease patients, particularlythose with diabetes

To develop, implement and embed healthpromotion and well-being programmes

To identify, manage and support patients at riskof developing long term conditions, preventing adverse effects to patientshealth

To provide routine nursing care to patients asrequired in accordance with clinical based evidence

Experience with COPD / Asthma or an interest in developing skills

To request pathology services as necessary

To process pathology results as required

To provide chronic disease clinics, deliveringpatient care as necessary, referring patients to secondary/specialist care asrequired

To maintain accurate clinical records inconjunction with extant legislation

To ensure read codes are used effectively

To carry out routine treatment room duties i.e.cervical smears, childhood immunisations, travel vaccines

To maintain chronic disease registers

To chaperone patients where necessary

To prioritise health issues and interveneappropriately

To support the team in dealing with clinicalemergencies

To delegate clinical responsibilitiesappropriately (ensuring safe practice and the task is within the scope ofpractice of the individual)

To support the clinical team with allsafeguarding matters in accordance with local and national policies

To deliver opportunistic health promotion whereappropriate

Secondaryresponsibilities

Monitor and ensure the safe storage, rotationand disposal of medicaments

Participate in local initiatives, QOF andenhanced services to support servicedelivery and patient care

Support and participate in shared learningwithin the practice

Continually review clinical practices, respondingto national policies and initiatives where appropriate

Participate in the review of significant andnear-miss events applying a structured approach, i.e. root cause analysis (RCA)
