Job description

As a Diabetes Nurse, you will provide and maintain a high standard of diabetes management for Type 1 & type 2 patients, which includes diagnosis, monitoring & development of individual management plans, agreeing these as appropriate with the patient & other health professionals. Specifically in Diabetes management and desirably in CHD and HF; you will work strictly in accordance with specific practice guidelines & protocols. Working within the NMC Code of Conduct and professional boundaries, the post holder will deliver primary health care interventions and care. The post holder will support service development, the safe delivery of policy and procedures, and demonstrate effective professional nurse standards, advocacy, and performance.

Main duties of the job
  • Working independently as a Diabetes Nurse in type 1 & type 2 management. Including managingdiabetic reviews and taking appropriate action as per current guidelines
  • Assisting the general practice team withmonitoring, educating, and reviewing patients on the diabetic register
  • Provide general and specific health screenings tothe practice patients within agreed protocols with referral to other membersof the primary and secondary health care team as necessary.
  • Recognise health promotion opportunities and givebrief, focussed lifestyle advice using motivational interviewing techniqueswhich may include weight loss, diet, exercise, smoking cessation
  • Assist with nurse-run diabetic clinics and carryout clinical measurements and near patient testing in line with NICE guidanceand Quality and Outcome Framework indicators.
  • Recognise signs and symptoms that may indicateundiagnosed conditions that may exist alongside diabetes.
  • Work in accordance with Practice protocols toensure the safe and effective management of patients with Type 1 or Type 2Diabetes

Lakeside St Neots is in a lovely town location witharound 21,000 patients. We are a diverseand dynamic team of GPs, Pharmacists, Physios, Social Prescribers, PracticeNurses, Health Care Assistants, Nursing Associates, Emergency CarePractitioners and ANPs.

Job responsibilities

We are looking for an experienced and motivated Diabetes Nurse in Type 1 & Type 2 diabetic patients: to join our dynamic team.

Youll be involved in the smooth day to day running of the Diabetes Clinics for Type 1 & Type 2 patients. You will be supported by the nursing team and an experienced administrative team as well as having support through clinical supervision with a General Practitioner.

Experience
  • Experience in the management of Type 1 & Type 2 diabetic conditions in primary or secondary care.

  • Experience of working within healthcare governance systems and understands role and responsibilities for CQC and within risk management
  • Experience of using clinical systems & Microsoft Office software
  • Management of other long- term conditions
