Job summary
Central London Community Healthcare NHS Trust is seeking a Band 6 Community Diabetes Specialist Nurse to join the Brent Community Diabetes Team on a 9-month fixed-term contract to cover maternity leave.
You will deliver high-quality diabetes care in community settings, provide specialist assessments, and educate patients and professionals while supporting service development within a multidisciplinary team.
Qualifications
- Degree in Nursing or equivalent.
- Current registration with the NMC.
- Post-registration training in Diabetes (diabetes management diploma).
- Supervisory skills training.
- Post-registration training in Cognitive Behavioural Therapy.
- Experience in Diabetes.
- Experience in managing a clinical case load.
- Experience in the training of students.
- Excellent group education and presentation skills.
- Experience of audits/research.
- Experience supervising others.
- Experience in a MDT.
- Experience in community settings.
- Experience with behaviour-change techniques.
Responsibilities
- Provide specialist diabetes care to adults across GP practices, health centres and patients' homes.
- Assess complex patient needs and develop evidence-based treatment plans.
- Provide specialist diabetes advice, treatment, counselling and lifestyle support.
- Provide expert diabetes guidance to practitioners via virtual clinics, email and telephone.
- Work with the Community Diabetes Team and partners to ensure coordinated care.
- Refer patients to other professionals as needed.

An exciting opportunity has arisen for a Band 6 Community Diabetes Specialist Nurse to join our Brent Community Diabetes Team on a 9-month fixed-term contract to cover maternity leave.
Working as a key member of our supportive multidisciplinary team, you will help deliver high-quality, integrated diabetes care to patients within the community.
You will provide specialist assessment, treatment and ongoing support for people living with diabetes, working closely with patients, families, healthcare professionals and partner services to promote effective diabetes management and positive health outcomes.
The role will include delivering care within community settings and patients homes, maintaining high-quality clinical records and supporting the delivery of diabetes education to healthcare professionals.
You will also have the opportunity to contribute to service development, working with colleagues and service users to develop and evaluate evidence-based diabetes interventions that respond to the needs of the local community.
Provide specialist diabetes care to adults receiving oral hypoglycaemic agents, insulin, insulin conversion and other therapies across GP practices, health centres and patients homes.
Use clinical expertise and professional judgement to assess complex patient needs, develop evidence-based treatment plans and adapt care in response to changes in clinical condition.
Provide specialist diabetes advice, treatment, counselling and lifestyle support to adults with complex and diverse healthcare needs, promoting positive behavioural change and effective self-management.
Provide expert diabetes advice and clinical guidance to medical practitioners through virtual clinics, email and telephone consultations.
Work collaboratively with the multidisciplinary Community Diabetes Team and partner agencies, contributing to MDT meetings and case conferences to ensure coordinated and continuous care.
Use clinical judgement to identify when patients require additional support and make appropriate referrals to other healthcare professionals, specialist services and Social Services.
