Job summary

South and East Leeds General Practice Group hosts the Diabetes Matron role, a Leeds system-wide, part-time nursing leadership post embedded across multiple neighbourhoods. The Matron leads advanced clinical decision-making for complex diabetes presentations and contributes to workforce development and service improvement.

The post is 0.4 WTE with flexible working, aligned to Tier 3 MDT activities and collaboration with partner organisations within the Leeds Health and Care Partnership.

Qualifications

• Senior diabetes nursing leadership experience.
• Experience within MDT settings across acute and community care.
• Knowledge of diabetes management and complex case reviews.

Responsibilities

• Provide advanced nursing leadership within Tier 3 Clinical Oversight Group.
• Lead MDT activities and workforce development across neighbourhood diabetes nursing.
• Oversee complex diabetes decisions, insulin optimisation and care pathways across organisations.
