Job summary

Lawton House Surgery invites applications for an experienced Practice Nurse to support patients with chronic conditions, with a focus on diabetes. The role involves long-term condition reviews, health promotion and vaccinations, and working with GPs and the wider team.

The successful candidate will maintain clinical records, follow safeguarding and infection control procedures, and contribute to CPD and professional standards. UK registration (NMC) is essential.

Qualifications

• Registered Nurse with current NMC registration.
• Experience in assessing and managing patients with long-term conditions.
• Experience in diabetes care and management is highly desirable.
• Ability to work autonomously within a multidisciplinary team.
• Good clinical assessment, decision-making and organisational skills.

Responsibilities

• Maintain accurate clinical records and follow safeguarding, infection prevention and control procedures.
• Support colleagues and students; contribute to clinical quality standards.
• Manage own workload and work independently within scope of practice.
• Review and monitor patients with diabetes, including blood pressure, weight and BMI; provide diet/exercise advice.
• Identify patients needing further assessment or referral and ensure care follows guidelines.
