Job summary

HCRG Care Group is seeking a Diabetes Healthcare Assistant to join the Specialist Diabetes Team based in Devizes, supporting patients across community, hospital and domiciliary settings under the supervision of Diabetes Specialist Nurses.

Based across Chippenham bases with 22.5 hours per week, the role offers full training for Xpert courses, a group pension, wellbeing resources and ongoing professional development through The Learning Enterprise.

Qualifications

• NVQ Level 3 or equivalent qualification.
• GCSE English Grade A-C (or equivalent).
• Ability and willingness to undertake additional training relevant to the role.
• Previous Healthcare Assistant experience.
• Experience working within a nursing or healthcare environment.
• Experience using computers, mobile phones, apps and email confidently.
• Strong understanding of IT systems and the ability to learn new technologies quickly.
• Experience educating, coaching or teaching patients.
• Experience working with individuals across different age groups.
• Excellent verbal and written communication skills.
• Ability to work collaboratively within a multidisciplinary team.
• Professional, patient‑centred and empathetic approach.

Responsibilities

• Support the delivery of specialist diabetes services across community, domiciliary and hospital settings.
• Educate and train patients in the use of glucose monitoring systems, mobile applications and digital healthcare technology.
• Manage a delegated caseload, prioritising referrals for diabetes education and technical support.
• Deliver accredited Xpert structured education programmes for people living with Type 2 diabetes.
• Undertake delegated clinical duties under the supervision of Diabetes Specialist Nurses.
• Provide technical support for glucose monitoring devices, sensor insertion and data downloading.
• Maintain accurate clinical documentation and patient records in accordance with organisational policies.
• Participate in MDT working and contribute to high‑quality, patient‑centred diabetes care.
