Job summary

Liquid Personnel Ltd is seeking an experienced Adults Social Worker to join the Adults Services team in Devon, based at Bideford Community Hospital. You will support adults to maximise independence, wellbeing and choice, within a strengths-based, person-centred service.

Responsibilities include Carrying out Care Act assessments, complex assessments, safeguarding, Court of Protection work where required, and collaboration with health partners and multidisciplinary teams to deliver high-quality

Qualifications

• Must be registered with Social Work England.
• Experience in Adult Social Care.
• Strong knowledge of Care Act, Mental Capacity Act, Mental Health Act and safeguarding legislation.
• Experience completing assessments, reviews, care planning and risk assessments.
• Ability to manage a varied caseload and work autonomously.
• Excellent communication and partnership-working skills.
• Full UK driving licence and access to a vehicle.
• Essential Compliance: Enhanced DBS with Adults' and Children's Barred List checks.

Responsibilities

• Complete Care Act assessments, reviews, and care and support planning.
• Undertake complex assessments and contribute to Continuing Healthcare (CHC) processes.
• Apply the Mental Capacity Act, including capacity assessments and Best Interest decision-making.
• Identify and promote enablement and reablement opportunities.
• Lead and complete safeguarding enquiries, including complex safeguarding investigations.
• Work effectively alongside health professionals and partner agencies.
• Undertake Court of Protection work and community deprivation of liberty applications where required.
• Maintain accurate, timely, and reflective case records.
• Contribute to service development and continuous improvement initiatives.
