Band 7: 37.5hrs/week

We are looking for an enthusiastic, experienced and committed clinical pharmacist to join our clinical pharmacy team. You will work alongside the Advanced Pharmacists for Medicine, Surgery and Renal Services, the Medicine Management Technicians and the wider pharmacy and multidisciplinary teams to provide medicines management for in and out-patients.

Main duties of the job

The clinical pharmacy team is expanding provision to medicine, surgery and critical care. This rotational role provides a variety of opportunities to develop professionally into more specialist areas.

Our established programme covers Aseptic Services, Medicine, Surgery & Older People. We are proud to host and provide the only regional Renal Services & Renal Homecare placement.

Key activities

  • Management and review of in-patients requiring acute care across medicine, surgery and renal services
  • Development and review of guidelines to improve patient management and transfer of care between care providers
  • Drug expenditure analysis, financial reporting and support to the multidisciplinary team to complete individual funding requests, patient group directions and formulary applications
  • In renal services, provision of clinical support to the Homecare team including prescription initiation and review
  • Contributing towards Cost Improvement Programmes and management/review of in and out-patient medication
  • Review of incident reports and DATIX to improve processes and practice to ensure safer medicines use
  • Operational support to the dispensary service
  • Induction and training of doctors, nurses, technicians and pre-registration trainees
  • Supporting the induction, training and development of rotational pharmacists within the Foundation Training Programme

The post includes rostered weekend and bank holiday working (currently 1 in 8), a weekly late duty and participation in the pharmacy on-call service.

Person specification
Education, Qualifications
  • Master’s degree or equivalent in Pharmacy
  • Pharmacist having current registration with the General Pharmaceutical Council
  • Demonstrates the ability to deliver a clincial pharmacy service
  • Postgraduate qualification in clinical pharmacy or equivalent evidence (2 for Certificate)
  • Qualified as an independent non medical prescriber
  • A minimum of two years post-registration experience in hospital pharmacy practice
Experience
  • Experience of providing clinical pharmacy services
  • Demonstrates an awareness of patient safety concerns
  • Evidence of ability to influence prescribing practice
  • Experience in clinical audit and implementation of outcomes
  • Maintains a portfolio of practice
