Main Duties & Responsibilities

Coordinate assessments of individual service users and develop their service plans in collaboration with significant others.
Review, reassess, and update service plans to ensure they meet current needs.
Assign implementation and record-keeping of service plans to staff with appropriate skills or training.
Provide individual support as needed and assess the need for specialist support.
Facilitate service users to express their views and incorporate these into service planning.
Promote development of new skills through training opportunities.
Communicate information regarding service contracts, quality metrics, and complaint procedures to service users.
Maintain high standards of professional care at all times.
Foster positive relationships with families and carers, encouraging their participation in care plans.
