Deputy Lead Practice Nurse Development to Lead Nurse
The Deputy Lead Practice Nurse will support the Lead Nurse with delegated duties, including managing the practice nursing team comprising nurses, TNAs, HCAs, and Phlebotomists. The role involves overseeing clinical areas such as vaccination programmes, health promotion, chronic disease management, well women and men clinics, IPC, and supporting clinical policy review and implementation. The Deputy will assist in managing long-term conditions and providing evidence-based care, especially for elderly and housebound patients. As a senior multidisciplinary team member, the post holder will ensure effective delivery of nursing services to the registered population.
Main Responsibilities
- Patient Support and Clinical Services
The Deputy Lead Nurse will develop skills to deputise for the Lead Nurse, including: - Acting as the Infection Prevention Control (IPC) lead, ensuring compliance with IPC policies, audits, coaching, and training.
- Producing the IPC annual statement.
- Developing care management programmes for frail and elderly patients.
- Assessing patient needs and providing appropriate healthcare based on evidence-based practice.
- Implementing health promotion and wellbeing programmes.
- Managing chronic disease treatment plans and supporting at-risk patients.
- Providing routine nursing care in line with NICE and NSF guidelines.
- Managing vaccination campaigns and administering vaccines per PGDs and scope of practice.
- Reviewing medications, guiding patients on medication use, and coordinating travel medicine services.
- Performing phlebotomy and supporting chronic disease clinics.
- Maintaining accurate clinical records and ensuring effective use of coding and chronic disease registers.
- Participating in well woman and well man clinics, ensuring compliance with chaperone policies, and assisting GPs with minor surgery.
- Prioritising health issues, managing clinical emergencies, and supporting mental health assessments.
- Supporting medication use, liaising with external agencies, delegating clinical responsibilities appropriately, and supporting safeguarding and substance abuse policies.
- Delivering opportunistic health promotion and ensuring clinical equipment compliance.
Systems and Administrative Support
- Reviewing and implementing protocols, supporting service improvements, and ensuring accurate patient data recording.
- Supporting IT advancements and managing communication with patients and staff.
- Coordinating nursing team rotas, supporting team development, and reviewing clinical practices.
Team-working
- Promoting collaborative working, effective time management, and supporting team policies and training.
- Participating in incident reviews, ensuring effective communication, and recognizing diverse communication needs.
General Duties
