An exciting opportunity has arisen to join our UCLH@HOME team as we pilot a new Blood Transfusion pathway. We are at the forefront of integrated care, providing innovative and transformational services that best serve local communities.

Main duties of the job

UCLH@Home is an NHS service supporting patients to leave hospital once medically optimized and safe to be managed in the community, while remaining under the care of their consultant team. Your role will involve direct care delivery within patients' homes, primarily working within the new Blood Transfusion pathway. This role requires an innovative individual comfortable working flexibly.
