Job Opportunity: Dementia Liaison Nurse

We are seeking a Dementia Liaison Nurse based in Wirral, covering a 7-day service. Although primarily based at Arrowe Park Hospital, this role is community-oriented.

The position addresses issues faced by people living with dementia and their carers. The nurse will aim to improve dementia care, support carers and professionals, and enhance primary care's understanding and capacity regarding dementia.

The successful candidate will work across Urgent Care Mental Health First Response Services, including Crisis Resolution Home Treatment Teams, Liaison Services, Street Triage, and more.

Main Responsibilities
  1. Work autonomously to deliver specialist assessments and interventions outside of mental health settings.
  2. Engage in care planning and collaborate with multidisciplinary teams and agencies.
