Dementia Liaison Nurse

An exciting opportunity has arisen for a Dementia Liaison Nurse based in Wirral, covering a 7-day service. Although based at Arrowe Park Hospital, the role is community-based.

The Dementia Liaison Nurse role is innovative, aimed at improving care for people living with dementia and supporting their carers and professionals. It also seeks to increase primary care's understanding and capacity to manage dementia.

The successful applicant will work across Urgent Care Mental Health First Response Services, including Crisis Resolution Home Treatment Teams, Liaison Services, Street Triage, Liaison and Diversion Services, and the Crisis Line.

Main Duties

The main duties include working autonomously to deliver specialist assessments and interventions in non-mental health settings, care planning, and collaborating with multidisciplinary teams to provide seamless services.

Salary range: £37,338 to £44,962 per annum, pro-rata.
