They will build caring relationships with residents and families, promote a culture that enables residents to maximize their capabilities, and work closely with the Activities Team to ensure the development of meaningful activities for residents with Dementia.

ABOUT THE ROLE

Your focus as a Dementia Care Manager will be to be responsible for the overall management of the Dementia Community, implementing and embedding the company's dementia care strategy and managing resources within a given budget.

Other responsibilities will include:

  • Ensuring that the plan care day is accurate and reflected in the care needs summary and that the resident/relative identifies needs and preferences. Ensure all information is integrated into a person-centred care plan.
  • Building caring relationships with residents and families, liaising with them sensitively and appropriately. Act as an advocate for the residents' best interests, appropriately managing conflict.
  • Working closely with the Activities Team to ensure the development of meaningful activities for residents with Dementia. Support and set expectations for the care team's participation in these activities.
  • Promoting a culture that enables residents to maximize their capabilities, optimizing their independence and enhancing their quality of life.
