The Dementia Team has a new exciting opportunity for a Dementia Associate Practitioner within our expanding team to support patients living with Dementia.
We are seeking highly motivated and enthusiastic individuals to work alongside the Dementia Team at Hampshire Hospital Foundation Trust, in Winchester and Basingstoke Hospital.

Main duties of the job
The candidate must have previous demonstrable experience in Dementia Care within a care setting. Excellent communication skills and a dynamic approach to delivering person-centred care to patients with Dementia, along with supporting their families and carers, are essential.
