re you passionate about transforming care for patients with dementia and delirium? Ready to drive forward innovation and excellence in clinical practice? We are seeking two, dynamic, forward thinking Ban 7 Clinical Nurse Specialists to join our team.
Advise on the care of people with dementia +/- delirium and their carers/families with complex care needs from peri diagnosis to bereavement support.
Improve identification and management of co morbidities and frailty for people with dementia +/- delirium.
Proactively review patients admitted with confusion from either dementia or delirium to assess cause of provide management advice to clinical teams
