This is a unique Dementia Adviser role working across Rotherham and Sheffield in the Rotherham Carers Resilience Service and our Sheffield Dementia Short-term Intervention Service and Hospital Discharge Pilot. These services offer specialist support for 4 to 6 weeks, providing advice and support, preventing crises, or offering crisis intervention support if needed.

Referrals for these services come from GPs in Rotherham and Sheffield and staff working in Sheffield Teaching Hospitals. No two days are the same in this role, and our work can truly transform the lives of people affected by dementia. You will support dementia carers in Rotherham and individuals with a dementia diagnosis (or on the dementia pathway), along with their families in Sheffield.

You will be supported by a small team of dementia advisers, the Sheffield & Rotherham local services manager, and the manager of the Carers Resilience Service in Rotherham.

About you

  • Be an empathetic listener, able to assess and evaluate client needs in a non-judgmental manner.
  • Able to work under pressure and manage your own caseload.
  • Be emotionally resilient and able to support service users to develop resilience.
  • Self-motivated, able to work independently from home using your initiative, but also a team player willing to support colleagues and work in community venues when required.
  • Have some knowledge or personal experience of dementia and its challenges.
  • Knowledge of relevant legislation such as the Mental Capacity Act, Safeguarding Adults, Data Protection.
  • Able and confident to travel within the service area and visit people in their homes.
  • Able and confident to organise and oversee group information sessions across the area.
  • Experience in record keeping and managing client databases, adhering to procedures and policies.
  • Good IT skills.
  • Able and confident to represent the needs of people affected by dementia at community events and meetings.

Interviews will be held on 8th May.

What you'll focus on:

  • Provide person-centred, outcomes-focused information, advice, and support to people affected by dementia and their carers.
  • Identify their needs and the services they may wish to access, giving advice on living well with dementia and supporting their wellbeing.
  • Work with professionals such as GPs, memory clinic staff, and other health and social care services.
  • Support may be provided in person, over the phone, virtually, or in writing.
  • Work towards making the service inclusive for all communities.
  • Work from home but spend time in the community for home visits, clinics, and dementia awareness talks, requiring comfort with public speaking.
  • Represent the service at primary care multidisciplinary meetings.
  • Participate in joint home visits with social workers when requested.
  • Attend awareness events and partner organization meetings.
