Job description

Are you passionate about improving healthcare services and driving positive change? Were looking for a proactive and organised Delivery Facilitator to support the implementation of key projects across our NHS Somerset teams.

Main duties of the job

You'll work closely with local teams and system partners. The postholder will provide day-to-day support and coordination, working with Primary Care team and newly forming Neighbourhood Health Teams to embed delivery and achieve success

  • Serve as a key contact and facilitator for GPSU workstreams, coordinating support and resources to ensure successful delivery.
  • Manage multiple projects simultaneously, tracking progress, identifying risks, and escalating issues as needed.
  • Promote learning and share best practices across system teams and partners.
  • Collaborate with system partners, VCSE organisations, community services, and the private sector to support wider transformation efforts.
  • Maintain accurate project documentation and contribute to business case development and reporting.
  • Support GPSU events, webinars, engagement sessions, and meetings, ensuring effective communication in collaboration with the Communications and Engagement Lead.
  • Identify opportunities for improvement and interdependencies with other local transformation projects.
  • Ensure compliance with GDPR and accessibility standards in all communications.
  • Stay up to date with NHS and public sector best practices, contributing to service improvement and innovation.
  • Take ownership of personal development and learning needs.
  • Attend relevant GPSU, Practice, and PCN meetings.
  • Be flexible to support occasional events outside normal working hours.
  • Support organisational learning through feedback and engagement insights.
