You'll work closely with local teams and system partners. The postholder will provide day-to-day support and coordination, working with Primary Care team and newly forming Neighbourhood Health Teams to embed delivery and achieve success

Salary Band 6 equivalent,
Proposed hours 30-37.5 hours per week
Coordinate and support the delivery of transformation programmes Work closely with clinical and operational teams to ensure smooth execution Monitor progress, identify challenges, and help find solutions Champion innovation and continuous improvement in patient care
