Join our dynamic Acute Care at Home (ACAH) service as an Advanced Specialist Pharmacist. This role is crucial to developing and providing clinical pharmacy services within our Community Rapid Intervention Service (CRIS) and Virtual Ward services. Your expertise will ensure excellence in clinical, governance, and operational functions.
As an Advanced Specialist Pharmacist, you will be an integral part of the multidisciplinary team, playing a pivotal role in delivering clinical services to patients within ACAH. Your responsibilities will encompass all aspects of ward-based clinical pharmacy services, including medicines reconciliation on admission and discharge, optimizing patient medication, and advising on monitoring protocols. Depending on the patient’s needs, your involvement may include remote consultations or home visits.
We are seeking a dedicated professional with substantial post-registration hospital experience or clinical pharmacy expertise, particularly in GP practices or intermediate care settings. A Master’s Degree in Pharmacy or an equivalent qualification is essential for this role.
