Brunelcare is seeking a passionate carer to join their Discharge 2 Assess team. You will provide valuable support to individuals transitioning from hospital to home, fostering independence through personal care and motivation. Salary £13.89 per hour (£14.89 weekend rate) plus a £500 Welcome Bonus.
Hours: 35 hours per week.

The D2A (Discharge to Assess) service provides short-term, high-quality support to individuals who are well enough not to need an acute hospital bed but do need some extra support in their home. Your responsibilities will include:

Our D2A team covers Bridgwater, Highbridge, Burnham-On-Sea, Cheddar, and surrounding areas, supporting clients discharged from hospital to recover at home.
