Job summary
Are you a Physiotherapist looking for a fantastic community opportunity where you'll be supporting people back to or within their own homes?
We are seeking a Physiotherapist to join our friendly D2A service, part of the wider Intermediate Care service for Somerset. This role offers an in-depth understanding of community working and supporting individuals within their homes.
Due to positive outcomes and progress, we have secured further investment to expand our service, resulting in several new posts available.

Note: As part of the selection process, a face-to-face interview with a practical component will be required. Please inform us of any accessibility needs or adjustments required, and we will accommodate accordingly.
You will be responsible for conducting specific therapy assessments, including goal setting, treatment planning, risk assessment, and promoting independent functioning. You should be committed to a person-centered approach, able to prioritize tasks, and work flexibly alongside provider teams and rehabilitation practitioners.
Are you a fully qualified Physiotherapist with active HCPC registration?
