About the Role:

The D2A service provides short-term, high-quality support to individuals who are well enough not to need an acute hospital bed but do need some extra support in their home.

You will be required to assess customers’ needs to identify areas where support is needed to promote independent living and provide person-centred care.

Motivating customers and building their confidence so they can achieve the goals they have set and return to independent living.

You will be supporting customers with tasks around the home and with personal care such as personal hygiene, toileting, and dressing.

Shifts will be a mixture of early and late shifts between 7 am and 10 pm and alternate weekends.

About you:

Previous experience in caring for vulnerable adults is essential. Sensitive, caring, and passionate about delivering a first-class service to people within your local community.

Proactive and self-motivated with great initiative and excellent communication skills.

QCF Level 2 in Health and Social Care is essential.

A full driving licence and access to your own vehicle are required, as you will need to be comfortable with lone working and travelling between customers.
