Children and Young Peoples Continuing Care,covering Buckinghamshire, Oxfordshire and Berkshire West (BOB Integrated CareBoard) are looking for an experienced Childrens practitioner / professional witha range of managerial and clinical skills, and a working knowledge of the ChildrensNational Framework for Continuing Care.
We cover a large geographical area there willbe opportunity to have a primary base in one of our counties. Our teamprimarily work virtually however there is an expectation that members work inthe office once a week and attend face to face meetings with children and youngpeople on the CYPCC caseload.

The post holder will manage a team of health and social carepractitioners who undertake a range of tasks within the National Framework (NF)for Children and Young Peoples Continuing Continuing (CYPCC) pathway includingEligibility Assessments, Case Coordination, Personal Health Budgets andEligibility Reviews and for supporting elements of the Transformation Plan.
Working within the remit of CYPCC, the post holder will offerthe BOB wide CYPCC team clinical leadrship and support the delivery of aneffective and competent service that consistently delivers anindividual-focused service.
The post holder will be responsible for ensuring the operationalteams practice is compliant with national policy, best practice and the NF forCYPCC.
The post holder will have a good knowledge of the NF forCYPCC and will support service development, analysis and improvement of theteams practice.
They will support the AACC CYPCC leadership team to developthe service and provide management oversight, guidance, expertise and supportto the staff to ensure that CYPCC processes are correctly followed and safecare commissioned to meet needs and maximise patient safety.
This post will require good management and communicationskills and requires the ability to work in a multi-disciplinary way withcolleagues.
Deliver against objectives, achieving quality outcomes,prioritising own workload and working to tight deadlines.
Managing CYPCC practioners will require good and effectiveworking relationships with internal and external colleagues.
The post holder will be responsible for ensuring theirprofessional registration and mandatory training is up-to-date.
