Midlands Partnership NHS Foundation Trust is seeking a Community Mental Health Practitioner to assess and support children and young people with mental health needs in the community and their families. You will contribute to the MDT, manage risk, and help avoid hospital admission while promoting safe and effective practice.

The role involves flexible working within the team to support crisis and intensive intervention, with external liaison to professionals and carers as needed.

Formación

• Requires registration with NMC, Social Work England or HCPC.
• Experience delivering community mental health assessments and interventions.

Responsabilidades

• Assess and treat CYP in the community, coordinating care to avoid hospital admission.
• Work as part of the MDT to support crisis, brief intervention and intensive support.
• Provide advice and supervision to other healthcare professionals.

Educación

First Level Professional Nursing/AHP/Social Work/Therapy Qualification in Mental Health, Social Work, Nursing, AHP or equivalent
