Job summary
The All Age Continuing Health Care (AACHC) team are seeking a motivated, passionate, and enthusiastic Registered Nurse (RSCN/SCPHN) with a can-do attitude to join our dynamic team. This role offers the opportunity to work both autonomously and collaboratively in a multi-agency environment to support assessment and review processes.
We are looking to appoint a qualified nurse, with experience working with children and young people (CYP) with complex health needs, to deliver our NHS Continuing Care service across Birmingham and Solihull. The role involves assessing CYP mainly in community settings, such as their own homes or specialized facilities. The assessment is a crucial stage in determining if a CYP is eligible for NHS-funded care.
CYP are often referred for assessment due to significant changes in their care needs, necessitating NHS Continuing Care. We need a nurse capable of holistic care needs assessment, who is compassionate and supportive during potentially difficult times.
It is a clinically led, supportive team working with some of the most vulnerable CYP in our community. The role includes planning and reviewing care, working with colleagues across health and social care to achieve the best outcomes for eligible CYP.
