Job summary

The Confederation, Hillingdon CIC is recruiting a CYP Co-ordinator to support children, young people and families in accessing mental health and wellbeing services. You will work across Primary Care Networks, coordinating care between GP practices, CAMHS, education, local authority and voluntary partners.

The role focuses on care coordination, service navigation, pathway development and quality improvement to improve access and reduce escalation into crisis services.

Qualifications

• Written and spoken English at a high standard (GCSE or equivalent).
• Degree or equivalent Level 6 in a relevant subject.
• Able to work proactively and use own initiative.
• Ability to build relationships and work across organisations.
• Excellent organisational skills and ability to prioritise demands.
• Experience coordinating services or managing referrals.
• Experience with multiple agencies.

Responsibilities

• Coordinate referrals across multiple services.
• Receive, review and triage referrals into the CYP programme.
• Build collaborative relationships across organisations.
• Provide advocacy, tailored support and empower self-management within sessions.
• Coordinate onward referrals to appropriate services.
• Support children, young people and families to navigate local services.
• Develop individual support plans where appropriate.
• Maintain regular contact with families while awaiting support.
• Escalate safeguarding or clinical concerns appropriately.
• Represent the CYP Programme across Hillingdon.
