Job summary

You will lead the multidisciplinary transfer team, contribute to governance and training, and deliver essential clinical care with emphasis on patient safety and service improvement.

Responsibilities

• Triage, coordinate, and provide decision support for referrals and transfers.
• Receive referrals and allocate transfer resources or signpost alternatives.
• Follow standard operating procedures around referral handling and clinical pathways.
• Manage tasking of clinical transfer teams and maintain high quality clinical records.
• Provide clinical leadership to the multidisciplinary transfer team.
• Engage in governance, quality improvement, and service training activities.
• Deliver professional care and maintain currency with adult critical care practices.
