Job Summary
Working as clinical lead in Urgent Care Services Crisis Team.
- Has responsibility for the promotion and assistance in the maintenance of the highest attainable standards of nursing care provided by the teams.
- Provides senior clinical support and advice to the team members ensuring they are visible to all staff to provide clinical leadership.
- Has the lead role of coordinating the team from a clinical perspective across a 7-day week, including clinical supervision, group supervision and MDT reviews.
- Ability to work closely with the team focusing on projects identified by management ensuring improvement of service provision and practice
Main duties of the job
You will ensure that the care delivered is excellent and in keeping with the most up to date guidelines and best practice.
To work with carers/family members as necessary to provide a thorough bio psychosocial assessment of their needs. Carers and significant others will be involved where appropriate in collection of risk information and in the development of a management plan.
To take an advisory and supervisory role in partnership with the clinical team in managing complex cases.
To lead and present complex, sensitive, and challenging clinical and professional communication processes that may impact on service users, carers, staff, the team, the organisation, partner agencies and members of the public, where there are barriers to understanding.
Safeguarding the health and wellbeing of children and vulnerable adults, by being familiar with and adhering to Trust safeguarding policies and participating in relevant training.
To utilise problem solving and decision-making skills to support safe discharge planning.
To ensure effective partnership working and pathways between the crisis team and key services both internal and external.
Working as part of the Urgent Care leadership team with willingness to provide clinical leadership across the Crisis Team- including the associated urgent care teams (Home Treatment/Mental Health Hospital Liaison Team) when required.
Deliver clinical supervision, training and clinical development to team members.

- RNMH current professional registration with NMC ,BA Hons Social work/Dip Sw registered with SW England.
- Professional knowledge acquired through a degree supplemented by postgraduate diploma specialist training, experience, short courses or specialist training
