A leading healthcare service in Grimsby is seeking a skilled mental health professional to provide crisis intervention for young people. In this role, you will assess risks, develop safety plans, and support families. The position requires experience in mental health services, strong communication skills, and a collaborative approach to care. This is a full-time position with a shift pattern that includes weekends and on-call duties.
37.5 hours/week Shift pattern to include on-calls and weekends

The team provide a 24hr, 365-day including theoccasional weekend and on call, generous enhancements are paid for these. This role will require you to provide care toyoung people experiencing a mental health crisis. We are a 24hr, 365-dayservice working within a shift system including the occasional weekend and oncall, generous enhancements are paid for these. Providing assessment of riskand formulating safety plans and care plans with children, young people andtheir families. Supporting young people to remain at home as an alternative toan inpatient hospital admission.
Utiliseexcellent clinical skills and judgement to assess individual needs and provideappropriate interventions to manage crisis periods. Work closely with families,carers and partner agencies to collaboratively care plan for any furtherinterventions that may be required. This role will include team working andautonomous practice with responsibility for caseload management from firstcontact to discharge. Successful applicantwill have the opportunity to gain skills and knowledge from all areas ofthe wider CAMHS service.

Toprovide a 24/7, 365 day crisis andintensive home treatment service to children and young people and families whoare presenting with acute mental health or emotional disturbance to a degreewhere the risk they pose indicates that an inpatient admission may be required.This includes those presenting with a significant eating disorder. In addition to this, the service wouldrespond to mental health crises in children and young people out of officehours
ProvideCare Co-ordination, needs assessment and risk assessment, planning, andimplementation of evidence-based care to a defined group of patients and theirfamilies/carers within the service catchment area.
Toprovide an intensive home treatment service to prevent admission to inpatientbeds and to facilitate early discharge.
Toprovide professional advice and support to other teams, agencies, and membersof the multi-disciplinary team.
Toprovide clinical and managerial supervision
Toprovide a high standard of clinical care ensuring safe and effectiveassessment, treatment and comprehensive discharge planning using highlydeveloped clinical reasoning skills and appropriate assessment tools
Providerapid response to children/young people presenting with urgent and emergencymental health presentations over a 24/7 rota including on-call duties, out ofhours
Actas Care Coordinator, as required
Participatein the On-Call rota on a regular basis
Engagewith young people to provide mental health and risk assessment, riskmanagement plans, and takingappropriate therapeutic risk for young people following crisis
Providefull assessment and appropriate follow up care via care planning, individualand group interventions, in partnership with young people/carers and otherworkers involved in care delivery.
Workin accordance with relevant childcare, health legislation and professionalstandards in partnership with wider children's services and to engage fullywith Safeguarding, Common Assessment Framework and other processes whichrequires working together with other agencies
Proactivelyinvolve children/young people and their families/carers in service developmentincluding using service questionnaires to obtain structured feedback.
Providea professional, non-stigmatising service to children/young people andfamilies/carers.
Workclosely with other colleagues within the service to enable effectivetransitions to and from other areas.
Ensurebest practice by incorporating NICE guidance and meeting healthcare standards.
Adhereto Navigo Policies in consideration of Children in Need and safeguarding issuesand in maintaining accurate and effective written records.
DevelopBand 5 staff and provide supervision/line management on all of the above.
Useoutcome measures to evidence effectiveness of treatments provided.
Participatein management and clinical supervision.
Activelycontribute to the ongoing development of clinical practice within the team.
Activelyparticipate in the delivery of training opportunities for staff and students.
Tobe able to travel independently and work flexibly.
Managecase-load and service delivery in accordance with, and in order to contributetowards the attainment of, the Service / Navigo productivity requirementsincluding waiting times.
Participatein all clinical governance and audit developments including post registrationeducation, training and continuing professional development.
