Job description

Job Summary

The post holder will be an expert clinical practitioner working in collaboration with the Coronary Heart Disease Specialist Nurse team to deliver a nurse-led, evidence-based Secondary Prevention Service, aligned with patient needs, local guidelines, and the National Service Framework.

Responsibilities include coordinating service delivery, development, and improvement, acting as an expert resource to ensure accessible support and advice for patients, relatives, carers, and staff. The post also involves representing South Tees Hospitals NHS Foundation Trust externally and developing regional policy.

All staff are expected to undertake additional tasks as required, including cross-cover within the department, and to adhere to Trust policies and procedures. The role may be amended following consultation with the post holder. A valid vehicle for work purposes is essential.

Main Duties
  1. Deliver holistic, Secondary Prevention Clinics across Redcar and Cleveland.
  2. Provide training to primary healthcare team members, including students and registrars.
  3. Offer professional development and training through presentations at trust and external educational events.
  4. Provide specialist assessments in Coronary Heart Disease, influencing service strategy and policy development.
  5. Collaborate with external agencies to ensure seamless care delivery across primary and secondary care.
  6. Engage in structured clinical reflection, appraisal, and personal development planning.
  7. Maintain up-to-date knowledge of Coronary Heart Disease and national policy changes.
  8. Act as a specialist in Coronary Heart Disease in primary care, improving patient understanding and management.
  9. Build and maintain effective communication networks with external agencies.
