A leading healthcare provider is seeking an expert clinical practitioner to join their Coronary Heart Disease Specialist Nurse team. The role involves delivering a nurse-led Secondary Prevention Service, coordinating service delivery, and providing training. The ideal candidate will have a nursing degree and experience in cardiac rehabilitation.
The post holder will be an expert clinical practitioner working in collaboration with the Coronary Heart Disease Specialist Nurse team to deliver a nurse-led, evidence-based Secondary Prevention Service, aligned with patient needs, local guidelines, and the National Service Framework.
Responsibilities include coordinating service delivery, development, and improvement, acting as an expert resource to ensure accessible support and advice for patients, relatives, carers, and staff. The post also involves representing South Tees Hospitals NHS Foundation Trust externally and developing regional policy.
All staff are expected to undertake additional tasks as required, including cross-cover within the department, and to adhere to Trust policies and procedures. The role may be amended following consultation with the post holder. A valid vehicle for work purposes is essential.
South Tees Hospitals NHS Foundation Trust offers leadership and improvement training for new staff, including courses on leadership impact, team success, service improvement, and lean practices. The programme aims to develop leadership skills, promote trust values, and equip staff to lead service improvements, with further training opportunities available post-completion.
