Job summary

Oxfordshire County Council is seeking a role focused on providing personalised information and advice under the Care Act (2014). You will manage a caseload, undertake community visits, and refer cases to the appropriate professionals in discussion with a practice supervisor.

A strength-based approach will explore informal support, community resources, and equipment assessments to support independence. You will conduct assessments, reviews and interventions for individuals, families and carers,

Qualifications

  • Excellent written, face to face and telephone communication skills.
  • Maintain comprehensive and accurate records.
  • Work within the Data Protection Act, confidentiality and information sharing protocols.

About the role

Our work is underpinned by the Care Act (2014) which has the wellbeing principle' at its heart. You will provide personalised information and advice to individuals, families, and their carers. Be responsible for the management and prioritisation of a caseload as allocated by the Practice Supervisor and to refer cases, in discussion with the supervisor to an appropriate professional. Use a strength-based approach to ensure that in responding to eligible needs all options including the use of informal support networks, community resources, and the assessment of equipment, adaptions and Occupational Therapy are explored to achieve identified outcomes. Undertake assessments, reviews and interventions and other duties relating to individuals, families, and carers. Ensure that support plans are reflective of the individual's needs Identify and make the necessary arrangements for services which will improve and maintain independence. Make telephone enquiries, undertake face to face visits in the community, facilitate clinics. Participate in duty including telephone and emergency advice. Identify and appropriately report any safeguarding concerns.
