Job description

The post holder will be responsible for assessing individuals in the community, home, nursing homes, and hospitals to determine eligibility for NHS Continuing Healthcare or Funding Nursing Care. The assessor will act as a member of the multidisciplinary team to coordinate the process, establish eligibility, and assist in identifying suitable care packages. The assessor will ensure that the review process for all NHS Funded Care is conducted in accordance with current guidance.

The practitioner will utilize national assessment tools and adhere to required timescales, coordinating, participating in, and supporting the assessment process with the multidisciplinary team. The role involves determining individuals' eligibility in line with the NHS Framework for NHS Continuing Healthcare and Funded Nursing Care, explaining the process to patients, families, health and social care colleagues, and care providers, ensuring transparency and understanding among all parties.
