Job summary

Medinet Group is seeking a Continuing Care Team Leader to oversee the Continuing Care team, ensuring patients receive a seamless experience after titration. You will manage prescriptions, annual reviews and monitoring while liaising with GPs and clinicians to maintain high-quality care.

You will lead a team of Coordinators and Administrators, drive service improvements, and ensure governance, data protection and records accuracy across clinical systems.

Qualifications

• Proven experience leading a healthcare administration or patient support team.
• Strong understanding of patient pathways and operational service delivery.
• Excellent organisational and stakeholder management skills.
• Experience using patient management systems (EMIS Web, Hero Health).
• Knowledge of information governance and GDPR.

Responsibilities

• Oversee the day-to-day management of the Continuing Care team and ensure safe, efficient care.
• Lead to ensure prescriptions, annual reviews and monitoring are completed.
• Act as escalation point for operational issues and drive service improvements.
• Maintain high-quality patient and GP communications and records.
• Collaborate with clinicians and external stakeholders to deliver care.
