A leading healthcare provider is seeking a full-time consultant in Diabetes & Endocrinology to join a dynamic multidisciplinary team. The position involves delivering outpatient and specialist clinic services, contributing to acute medicine, and providing educational supervision. Applications are invited for a full time consultant post in Diabetes & Endocrinology. The successful candidate will join a team of 7 consultants at Mid Yorkshire Teaching NHS Trust, and may have commitments at any of our 3 sites.
The multidisciplinary team at Mid Yorkshire is dynamic and consists of Consultants, doctors in specialty training posts, Diabetes Nurse Consultant, Diabetes Nurse Specialists, Endocrine Nurse Specialists, research nurses, podiatrists and dieticians. In addition to general diabetes & endocrine services, there are a range of speciality services including, diabetes and endocrine antenatal services, diabetes foot services, insulin pump clinics, young adult clinic, transition clinics, weight management services, radio-iodine treatment.
The successful candidate will have the opportunity to develop their own specialist interest, contributing to the range of services currently provided at Mid Yorkshire.
Our consultants also staff an inpatient Diabetes & Endocrinology ward on a rotational basis.
The post holder will join the out of hours Acute General Medicine Rota, this involves evening (1in 16) or weekend (1 in 8) ward rounds on the acute medical unit at Pinderfields hospital and on call based from home (1.5 PAs).
Main duties of the job

A range of general and specialist outpatient services in Diabetes and Endocrinology
Contribution to Acute Medicine on call and General Medical inpatients
Diabetes & Endocrinology outpatient clinics, MDT meetings and supporting Diabetes inpatient team.
Administrative duties to support outpatient and inpatient work (office space and secretarial support will be provided)
Supporting Professional Activities in accordance with the Trust Framework with at least 1.5 SPA (core) and such additional PAs (DCC or SPA) offered according to roles and duties commissioned.
On-Call duties for general medicine currently assessed as 1.5 PA (1:14 weekdays, 1:8 weekends)
Educational supervision for junior doctors ( from FY1 to Specialist Registrar). These will be additional SPA and job planned as 0.25 PA per trainee (1 hour/ week).

This substantive post is offered on a full-time (10 PA) basis although less than full-time applications will be considered. Duties will be based primarily on our Pinderfields site, although all our medical staff are involved in delivering care to patients on all our three sites including Pontefract and Dewsbury Hospitals.)

Consultants presently work on a 1:6 rotational basis on Gate 32 which is the Specialist Diabetes & Endocrine ward at Pinderfields Hospital, the 2 timetables below reflect the work patterns when on or off the specialist ward. With the appointment to this post/s the ward rotational frequency will change to 1:8.
When on ward cover, you are expected to do daily ward rounds on Gate 32 and provide consultant supervision to Gate 32 A Medical outliers (this is primarily a ENT, Maxfax and orthopaedic ward). The workload is variable and usually not more than 5 patients.
Gate 32 has 23 beds including 3 bays and 8 side rooms. A morning handover occurs with the team, nursing staff and therapist and the consultant on the ward. There are usually 3-4 junior doctors who are from FY1-2 level, trust grade registrars, IMT and specialist registrar level supporting Gate 32. You will support and advise registrars and nurses to provide a speciality in reach services as needed to gate 12 ( Medical Admissions Unit ) and other wards in the hospital. Referrals are made via the Alertive App and consultants on the ward have a consultant and consultant function to contact consultant colleagues from other specialities for advice. We use PPM+ which is an electronic notes system allowing easily review of notes and management plans as well as E-meds for electronic prescribing.
The Diabetes and Endocrine team are an established multidisciplinary team consisting of Consultants in Diabetes & Endocrinology, a Nurse Consultant in Diabetes, Diabetes Matron, Specialist Diabetes Nurses , Endocrine Nurses, Clinical Assistant in Endocrinology, Specialist Dieticians, Clinical Psychologists, Specialist Podiatrists and Research nurses. There are close links with paediatrics, vascular, adrenal, obstetric and thyroid surgery departments within the trust. Additionally, we have a close links with the Thyroid Cancer MDT, Pituitary and Endocrine Surgery Teams at Leeds and Sheffield University Hospitals as well as regional NET centres at Leeds and Sheffield.
We have an established Tier 3 and Tier 4 weight management services serving the local community. There is an established MDT comprising bariatric surgeons, Bariatric physician, psychology, physiotherapy and dieticians.
The Consultants undertake a range of specialty clinics, which include MDT Diabetic foot, lipid clinics, bone and metabolism, antenatal diabetes, insulin pump and young adults besides for transitional diabetes and Endocrine clinics with Paediatrics. There is a large and growing population of patients on hybrid closed-loop insulin pump therapy. Additionally, since 2009 we have a well-established primary care diabetes integrated service which provides support and guidance for local GPs and practice nurses through joint specialist clinics, case note reviews and formal education sessions. We also provide specialty support to HMP Wakefield on a sessional basis.
The consultants also provide a e-consultation service, for advice and guidance with an in-built triage service to organise tests and outpatient appointments as felt appropriate.
Additionally, for the first three years following appointment, all new substantive appointments will provide consultant input to a General Medicine ward (Gate 44) to enable the successful applicant to be part of the team delivering inpatient care to General Medical inpatients. This ward will be staffed on a rotational basis by newly appointed dual-accredited physicians from Group 1 specialties. These consultants will work a consultant of the week model of care on a one-in-eight rota, providing a daily consultant ward round (4-hour duration). They will be supported by a team of residents. The ward will have 2-3 consultants each week doing a daily ward round, the consultants will change every week.
On call duties are for Acute General Medicine and based on the Acute medical Unit ( Gate 12) at Pinderfields Hospital. Resident on call duties consist of ward round 9am -5pm weekends and on call on either Saturday or Sunday evening (provide on call phone support 1700 -0900 and return if needed Category A 5%). When on call you on weekends, you are rostered with another GIM/Acute physician. Respiratory, Cardiology and Elderly Medicine have their own speciality takes for admissions. You are supported by a team of junior doctors including Specialist Registrars, IMT, FY1-2 and Physicians Associates. After completing a full weekend, you will have 2 days off as rest days on the Monday and Tuesday the following week.
Weekday evening ward rounds on call are 1700-2100 and same overnight support as per weekends. You do not get time off the following day after a weekday evening on call.
On calls for medicine attract a 1.5PA allocation and Category A at 5% on call supplement.
The department also provides a popular undergraduate placement for 5th Year Medical Students from Leeds University School of Medicine. You would be expected to participate in medical student teaching as well as completing their assessments.
We have a number of residents in the team including Speciality trainees, Trust Grade Registrars, IMT, Gateway and FY1-2 doctors. There is an expectation that the consultants will provide clinical supervision to these doctors, as well as Educational Supervision for trainees. Newly appointed consultants will be expected to complete locally available training to become Educational Supervisors.
