Job description

We are looking for a Consultant ACP to join our Trust supporting site across East and West Kent.

The Consultant Practitioner for Community Hospitals co-leads the medical provision for Community Hospitals, alongside the Consultant Clinical Lead.

This includes medical management of community hospital services across east and west Kent; development of the current and future medical model; and participation in service redesign.

It would be particularly suitable for a forward thinking, innovative individual seeking an exciting opportunity to help plan and deliver modern dynamic Inpatient Frailty services.

You will be a clinical expert in Frailty and will fulfil the 4 domains of Advanced Clinical Practice:

  • Advanced and expert level clinical practice within the field of frailty ensuring clinical excellence
  • Leadership and consultancy providing expert professional advice to patients, carers and colleagues
  • Development and provision of education and training to other staff and trainees
  • Service development, evaluation, research and audit.
