A leading organization seeks a Complex Needs Navigator to engage with rough sleepers in Croydon. The role involves coordinating care, developing personalized action plans, and collaborating with various agencies to reduce street living while ensuring access to essential services. Ideal candidates will have experience with complex needs, strong communication, and a proactive approach to problem-solving.
Contract Duration: 1 Year Fixed Term
We are looking for a Complex Needs Navigator to work with rough sleepers who are the furthest from support, accessing services and care act needs as required within Croydon's T1000 cohort.
You will focus on engaging with people who have a history of rough sleeping and substance misuse. You will have some experience of engaging with individuals with multiple and/or complex needs and demonstrate a determined approach to finding solutions.
About the role
· Coordinate and lead a solution-based approach, engaging Service Users in appropriate services moving them away from street-based living
· Carry a caseload of complex clients, developing personalised action plans working towards discouragement service will provide wraparound support to rough sleepers and will work directly with emergency accommodation to support the staff with this group of people.
· Participate in multi-agency initiatives to reduce the level of street activity
· Work closely with social care services, mental health teams & drug and alcohol teams
· Work proactively to ensure that Service Users have access to ongoing healthcare and seek to relieve rough sleeping altogether with housing led solutions
You will be part of a commissioning team that deliver various services and will be part of wider team strategies involving rough sleeping delivery
About you
· Experience of working with rough sleepers with substance misuse, alcohol misuse, mental health issues and complex needs.
· Knowledge and experience of multi-agency working with experience of facilitating multi-agency meetings
· Excellent communication, negotiation, and advocacy skills
· Be able to build and maintain relationships with a wide range of services
· Can work autonomously as well as part of a team
· Good levels of IT literacy, including the ability to use IT systems including MS Office applications
