Job overview
We have an opportunity for a Band 7 Complex Care Nurse to join our Complex Care Nursing service in Tunbridge Wells.
Our complex care nursing service provides proactive and responsive care to support patients with frailty and other long-term conditions such as Heart Failure, COPD, and diabetes.
We support patients with management plans to promote self-management, optimize long-term condition management, and prevent unplanned hospital admissions.
Main duties of the job
Each nurse manages a defined patient caseload within an area, caring for patients of various ages, often elderly, recently discharged from hospital, terminally ill, or with disabilities.
As a Band 7 Complex Care Nurse, you will work as part of a team to ensure patient safety and well-being in Maidstone, using clinical judgment for evidence-based care planning to ensure positive patient experiences.
Additional Working Pattern: 3 days/week, 1 weekend in 4.
