Job description

Opportunity: Band 7 Complex Care Nurse - Maidstone

Join our Complex Care Nursing service in Maidstone, providing proactive care to support patients with frailty and complex long-term conditions such as Heart Failure, COPD, and diabetes.

Our service promotes self-management and aims to optimise long-term condition management while preventing unplanned hospital admissions. Each nurse manages a defined patient caseload within an aligned geographical area.

Patients vary in age; many are elderly, recently discharged from hospital, terminally ill, or have physical disabilities.

Role Responsibilities

• Work as part of a team to keep people safe and well in Maidstone.
• Use clinical judgement to support evidence-based care planning.
• Ensure patients have a positive care experience.

Additional working pattern: 4 days/week, 7.5 hours/day, with 1 weekend in 4.
